The following categories require a CPNI release form and Signed AOL form in order for Verizon Wireless to be able to release your billing information to DMS/DivTel. Please complete the online CPNI release and submit the AOL form (link on the right) to your Verizon Wireless Sales representative, in order to begin your account migration to the new State Contract terms.
